Payment Policy

At Chandler Family Dentistry, we value honesty and transparency with our patients about costs. Our aim is to help you understand treatment costs and the payment process. We provide clear information on fees, insurance, and payment options, so you can make informed choices about your dental care. *FULL PAYMENT (including deductibles and copayments) IS DUE AT THE TIME OF SERVICE.

*WE ACCEPT CASH, CHECKS, CREDIT CARDS, CARE CREDIT and Sunbit.

Patients with Insurance

We provide insurance company billing as a courtesy to our patients. Estimated patient portion is due at the time of service and may be adjusted after the insurance claim is processed. Note that some insurance plans have annual limits for dental services. If patient services exceed these limits, patient is responsible for the excess costs. It is the patient’s responsibility to monitor remaining benefits for the year. The patient may not rely solely on information provided by our team regarding your benefits.

Payment in full is required on the day of treatment for services not covered by insurance. We can assist with insurance questions and billing issues, but patient insurance contract is between the patient and their insurer. If patient insurance does not pay, patient is financially responsible for the amount due.

Patients without Insurance:

Payment is required on the day of treatment. For full payment issues or multi-visit treatments, an invoice will be issued, and payment arrangements can be discussed with our business office staff.

Discount for Cash Payment:

Our practice offers a 5% discount for full payments made in cash at the time of service.  Exclusions apply to services covered by insurance or partial payments. In order to receive discount patient should inform our front desk at checkout. 

Cancellations/No Show Policy:

We strive to see patients on time and request 48-hour notice for cancellations or rescheduling. The first missed appointment without proper notice is free, but a $50 fee applies for the second. The third time, a $75 fee is charged for allotted appointment time. Please understand any changes in our schedule affects patients waiting to complete their dental care.

Collections: 

Late payments incur a fee. If a payment agreement is broken, the account may be sent to a third party or collection agency.
